Stay Up to Date on the Latest Kidney Supportive Care Research Group Events

The theme of this year's symposium is Connections Across the Care Continuum. This event will bring together attendees from diverse healthcare disciplines/settings and others who have an interest in the prevention and management of kidney disease. The conference will include a workshop stream showcasing Conservative Kidney Management and it's impact on patient care.

Celebrating Achievements

Alberta Health Services (AHS) Great Teams. Great Care Award

We are proud to announce that four Northern Alberta Renal Program (NARP) Chronic Kidney Disease (CKD) clinics piloting the Conservative Kidney Management pathway were recognized as one of the successful applicants of the 2017 AHS Great Teams. Great Care Award for their work on the Conservative Kidney Management.

This video was featured on Alberta Health Services website to highlighting the clinic staff's diligent work to improve patient outcomes.

Conservative Kidney Management (CKM) Innovation Learning Collaborative Workshop Series

Four kidney clinics in Northern and Central Alberta piloted the CKM pathway. The Kidney Supportive Care Research Group in partnership with the Kidney Health Strategic Clinical NetworkTM hosted a series of workshops to allow pilot sites to share their success stories, receive additional training, and engage in trouble shooting.  Over 60 participants from the CKM pathway pilot kidney clinics and healthcare professionals from a variety of care settings attended these sessions. These workshops were also open to those patients, family members and  community care providers interested in learning more about conservative kidney management.

Clinics were supported in the creation of practical CKM Pathway implementation plans, and quality improvement scorecards.

Workshop #2

Date: Apr 6, 2017

Participants were introduced to the interactive Patient Decision Aid developed around the appropriate initiation of CKM versus dialysis.

Date: Nov 21, 2017

Clinics celebrated their achievements, and shared their success stories, scorecards and action plans for sustainability.

World Cafés

Participants from the Conservative Kidney Management (CKM) Pathway pilot sites, healthcare professionals from a variety of care settings and patients attended this learning session. Education was provided on the following topics:

Clinical Pathway Training  (Sept 13, 2016)

  • The choice for Conservative Kidney Management
  • Symptom management and Chronic Kidney Disease guidelines for CKM patients.
  • Available community partners and how we can work together to optimize care.

Advanced Care Planning & Patient Decision Aid (Feb 10, 2017)

  • Advance care planning skills training to engage with patients in these crucial conversations
  • A demonstration and feedback discussion about the new CKM Patient Decision Aid tool.

A World Cafe was hosted in both Edmonton and Calgary to identify current challenges, enablers, and opportunities to providing excellent care for patients who choose conservative kidney care. We engaged with stakeholders across Alberta from diverse healthcare disciplines, settings, and with patients and families.

"Disruptive Innovation" Market Fair

The KSCRG was pleased to be accepted to the annual APCC. The conference's theme, Advancing Patient Collaborative Care, focused on three areas: collaboration, integration and innovation.

In conjunction with a submission to AHS's Great Teams.Great Care competition, the KSCRG created a poster to acknowledge the dedicated work of the kidney clinics that piloted the CKM Pathway.

The Kidney Supportive Care Research Group was excited to exhibit the innovative Conservative Kidney Management Pathway at the March 21, 2017 Alberta Health Services Senior Leaders Meeting.
