Conservative Kidney Management World Cafes

Fostering Collaborative Dialogue and Active Engagement

Dr. Sara Davison and her team received a three year grant to develop a Provincial Conservative Kidney Management (CKM) Pathway to provide sustainable, high-quality, evidence-based care for patients unlikely to benefit from dialysis. Implementation of the pathway was piloted in four Chronic Kidney Disease Clinics in the Northern Alberta Renal Program. To identify current challenges, enablers, and opportunities to providing excellent care for patients who choose conservative kidney care the pathway the Kidney Supportive Care Research Group, in partnership with the Kidney Health Strategic Clinical NetworkTM, hosted a World Café in Northern and Southern Alberta (Sept 23 and Nov 12, 2015, respectively).

A World Café is an innovative and flexible method for hosting conversations about questions that matter. The process enables participants to interact and build relationships, gather and share experiences, explore issues, create shared visions and translate them into action steps.

The Conservative Kidney Management World Cafés involved 25 minute rounds of progressive conversation to address four key questions:

  • What makes providing quality conservative kidney care a challenge?
  • Taking the perspective of a conservative kidney patient (or family member), what challenges do you experience in making that decision, managing your care, and interacting with your care providers?
  • What is working well in terms of conservative kidney care?
  • What opportunities or ideas could be implemented to improve conservative kidney care?

Over 50 participants attended including patients/families and multidisciplinary stakeholders from a variety of healthcare settings.

All participants were assigned to one of four tables. After each round, participants switched tables to discuss a new question. Each table was assigned a stationary facilitator and host to introduced the question for discussion, ensure each person had the opportunity to participate and share ideas generated from the prior conversations. At the end of four rounds, key insights and themes were shared with the entire group.

Input from the World Cafés was used to support the development, implementation, and evaluation of the provincial Conservative Kidney Management (CKM) pathway.
