2018 Alberta Kidney Days Conference

Sharing our Experience Developing and Implementing a Conservative Kidney Management Pathway

The theme of the 2018 Alberta Kidney Days conference was Connections Across the Care Continuum. The conference, hosted by the Kidney Health Strategic Clinical NetworkTM, brought together attendees from diverse healthcare disciplines/settings and others who have an interest in the prevention and management of kidney disease to provide learning, networking and knowledge translation opportunities. The conference included a workshop stream showcasing Conservative Kidney Management (CKM) and it's impact on patient care.

Conservative Kidney Management is planned, comprehensive, person-centred care for patients with end stage chronic kidney disease for whom dialysis is unlikely to provide benefit based on informed and shared decision making. A CKM Clinical Pathway was developed to provide sustainable, high-quality, evidence-based care for patients who choose conservative (non-dialysis) care and piloted in four Northern Alberta Renal Program Chronic Kidney Disease (CKD) Clinics.

Stakeholders from across Alberta such as, Nephrologists, Clinical Nurse Educators, frontline CKD clinic staff, and patients and their family members came together to share their experience with the Conservative Kidney Management pathway.

The CKM stream covered several important topics such as:

  • Shared Decison Making and the Choice for Conservative Kidney Management
  • Patient and Caregiver's Experience with CKM
  • Development of the interactive web-based CKM pathway tool
  • Implementation of the CKM pathway
  • Integrating Kidney, Primary and Palliative Care
