Reporting and Correcting
includes these two sub-elements:
1) Observing and Resolving
2) Reporting Incidents
Observing
and Resolving:

Students and Staff
are expected to take a proactive approach for improving safety in our
laboratories.

Students and
Staff are expected to continually observe their workplace laboratory for
sub-standard conditions or at-risk work practices.

When such
sub-standard conditions or at-risk work practices are observed, they are
expected to take steps to address and resolve the sub-standard condition or
at-risk work practice.

If immediate
correction is not possible and requires more extensive follow-up, alert Your
Lab Supervisor and seek help to resolve the issue.
Reporting Incidents

Staff and
Students MUST report any Near Miss Incident (a close call, someone was almost
injured or the equipment almost failed), or any Loss Incident (a person was
injured, or equipment was damaged, or equipment malfunctioned) to the Principle
Investigator. This also applies to
property damage caused by a problem or failure in the building. A recent example is if a pipe going through
your lab starts leaking and the water damages equipment lab equipment. The EH&S staff that get the reports will
forward it to the parties concerned.

The Principle
Investigator shall investigate to a suitable degree, all Near Miss and Loss
Incidents.

If the incident
meets the
reporting requirements from UofA EH&S, then the Incident must be
reported using their
WEB Page. If there is any question
on whether an incident should be reported use their ASK
EHS Link.

If you go to go
to a Doctor for an injury that happened at work there are extra Procedures.

The Principle
Investigator shall be responsible for resolving all corrective and preventative
actions found from the incident investigation.
