| Location The Fairmont Chateau Lake Louise, Lake Louise, Alberta is a modern hotel with an old world ambiance nestled on the shores of Lake Louise and located next to one of Canada's largest ski areas with runs for all levels of skiing ability. The Chateau is a mecca for winter activities including dog sledding, snowshoeing, horse-drawn sleigh rides, skating, downhill skiing and miles of cross-country skiing or you can relax and enjoy one of the most scenic vistas in the world. Preferred room rates are available at the Fairmont Chateau Lake Louise (Quote Canadian Rheumatology Association Conference): Bedrooms start at $195.00 (Canadian Pacific Mountainside) per night single/double occupancy PLUS a charge of $9.00 per adult per day to cover porterage, housekeeping and group services. A third person sharing is an additional $25.00 per night. In addition a 5% Alberta hotel room tax and 7% GST will be charged. Children 18 years old and under sharing their parents accommodation are complimentary. Gratuities for bellmen and maids are covered in the $9 per adult daily charge. NO TIPPING IS REQUIRED FOR THESE SERVICES. Reservations should be made by phoning the Fairmont Chateau Lake Louise

Extensions to check out should be directed to the Front Desk. A Dinner will be held on the evening of Friday, February 22, 2002. Tickets for CRA Members, Non-CRA Members and Trainees are available at a cost of $25 each if purchased BEFORE Feb 11th and $35 each if purchased AFTER Feb 11 or ON-SITE. Accreditation and Study Credits The Division of Continuing Medical Education at the University of Alberta is fully accredited by the Committee on Accreditation of Canadian Medical Schools (CACMS) and the Accreditation Council for Continuing Medical Education (ACCME) of the United States to sponsor continuing medical education for physicians. This education event is approved as an Accredited Group Learning Activity under Section 1 of the Framework of CPD options for the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada. Kids/ Teen Program If there is interest, we will develop a Kids & Teens Program for Wednesday, Thursday and Friday evenings. The Kids Program (5:30 to 8:30 pm) will include a Kids Buffet Dinner and activities such as Arts & Crafts, Story Corner,
board & Card Games etc. The Teens Program (6:30 to 9:30 pm) will include
Teens Buffet Dinner followed by activities such as Dance & Soda Bar,
Games night & Karaoke/Joke night. Parent Volunteers will be required.

Ski Packages (Pre & Post Conference) Ski information may be obtained by telephoning the Concierge's Desk at the Fairmont Chateau Lake Louise at 403-522-3511 (indicate you are with the Canadian Rheumatology Association Meeting) or by checking website www.sblls.com/ab CRA Annual Ski Race The 3rd Annual CRA Ski Race will take place on Friday, February 22nd from 1 to 3 pm. This great event is open to all meeting participants and families of all skiing abilities. Funds are raised for the Ogryzlo Award thanks to Industry Sponsorship. Lunch will be provided on the mountain-side terrace. Ski lift ticket for the race (ticket good for all day) are FREE. Please indicate on the Meeting Registration Form your interest in being part of this fun event. |
| Reimbursement of Registration
Fees (AMA) Physicians may be eligible for reimbursement of registration fees and expenses to attend this course from a fund administered by Provincial Medical Associations. Please call your local Medical Association for information. |
Cancellation/Refund Policy Cancellations must be received in writing along with the original receipt of payment before February 12, 2002 and will be subject to a $50.00 processing fee. No refunds will be considered after February 12, 2002. The University of Alberta reserves the right to cancel or postpone a course due to insufficient registration, or circumstances beyond our control. No refunds for Dinner Tickets - ALL SALES ARE FINAL |
