Requirements for Job Safety Analysis / Safe Operating
Procedures:
1) A Safe Operating Procedure must be conducted by the
researcher and approved by the principle investigator before any new
experiments are planned and conducted, or before any new equipment is used.
2) The Safe Operating Procedure can be created using the
Job Safety Analysis methodology and/or the manufacturer’s specifications
(installation instructions, operating instructions, equipment manual, etc.).
3) Laboratory Personnel not familiar with the experiment
or equipment shall be trained initially using the Safe Operating Procedure
before conducting the experiment or using the equipment.
4) Laboratory Personnel, who have not run the experiment
or used equipment within six (6) months, shall be re-trained using the Safe
Operating Procedure before conducting the experiment or using the equipment.
5) Laboratory Personnel shall follow the requirements of
the Safe Operating Procedure at all times.
6) If the Safe Operating Procedure for the experiment or
equipment has been established, the researcher shall review the SOP at least
once every year, or when the procedure has been altered in such a way as to
possibly introduce new hazards or degrade any control measures / safeguards.
7) The person who established, reviewed, or revised the
SOP shall be noted on the SOP, as well as the date that the person established,
reviewed, or revised the SOP.
8) The person who approved the original and any revisions
to the SOP shall be noted on the SOP, as well as the date of approval.
9) A list of all JSAs and SOPs shall be maintained by the
Laboratory Supervisor or P.I.
